Job Summary
The Hospice Clinical Liaison plays a dual role that combines clinical nursing expertise with strategic relationship-building. Serving as both an admissions nurse and a community liaison, this role is responsible for assessing patients for hospice eligibility, facilitating timely and compassionate admissions, and developing strong partnerships with referral sources to grow awareness and trust in our hospice services.
Key responsibilities
- Conduct timely and thorough clinical assessments to determine hospice eligibility based on established criteria.
- Collaborate with the interdisciplinary team to develop patient-centered plans of care.
- Educate patients and families on the hospice philosophy, services, and what to expect throughout their care journey.
- Ensure all admissions documentation is completed accurately, timely, and in compliance with federal, state, and agency standards.
- Maintain up-to-date knowledge of hospice guidelines, insurance policies, and community resources.
- Provide emotional support and guidance to patients and families during the transition to hospice care.
- Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, and community healthcare partners.
- Promote awareness and understanding of hospice services through education, presentations, and one-on-one interactions with referral sources.
- Actively identify and respond to new referral opportunities and coordinate closely with the intake and clinical teams for seamless transitions.
- Represent the organization in the community at health fairs, professional networking events, and referral meetings.
- Track and report referral patterns and outcomes, providing insight to leadership on trends and outreach opportunities.
Qualifications
- Current RN license in good standing, required.
- Graduate of an accredited nursing program (Associate’s or Bachelor’s degree).
- Minimum of 1 year of experience in hospice, palliative care, or related healthcare setting required.
- Previous experience in a clinical liaison, admissions, or healthcare sales role strongly preferred.
- Strong assessment skills and familiarity with hospice eligibility criteria (including Medicare regulations).
- Excellent communication, interpersonal, and organizational skills.
- Demonstrated ability to provide compassionate care and education during emotionally sensitive situations.
- Proficient in electronic medical records (EMR) and documentation practices. Homecare Homebase preferred.
Requirements
- Valid driver’s license, reliable transportation, and current auto insurance.
- Flexibility to travel locally and work a schedule that meets the needs of referral sources and patients.
- Ability to lift up to 50 lbs and manage physical requirements typical of in-home or facility-based clinical evaluations.
- CPR certification required.
